In the world of Chainsaw Man, Devils are born from human fears; they are usually dangerous, malevolent, and their power is proportional to the fear they inspire. Humans can make contracts with them to use their power, and Devil Hunters specialize in their elimination and use. Devils come from Hell, migrating to Earth when they are killed in Hell and vice versa. Inhabiting Hell are extremely powerful devils, called Prime Fears, who have never been killed.

A Devil can, under certain conditions, live inside a human body, acquiring part of their memories and personality; such a creature is called a Fiend. In rare cases, a dead human can maintain their personality and control over the body's balance with their Devil, with whom they are able to converse.

The story's events take place in 1997 in an alternate timeline where the Soviet Union still exists and many events like the Holocaust, AIDS, and nuclear weapons have been erased from history and ceased to exist entirely due to their respective Devils being consumed by the Chainsaw Devil, with only a powerful group of devils known as the Four Horsemen remembering the original events. Similarly, there has been no war since World War I because the War Devil has been weakened to the point that the only traces of war exist in pop culture and media.
