The game is set in a more futuristic, dystopian, and post-apocalyptic version of planet Earth, where magic exists and part of the population exhibits physical characteristics typical of animals or mythical creatures. Following some natural disasters, called Catastrophes, a mineral called originium appeared on Earth, which has two effects: it infects people with a disease called oripathy, and it enables or amplifies the use of arts (magic). Due to this latter property, the mineral is also used in energy and military fields. Since the disease has a 100% mortality rate and is highly contagious at the time of death, hostility towards the infected is common. Earth, similarly to the real one, is governed by various forms of power, which have come together to form a plan to quarantine the population and contain the disease on a large scale, sometimes resorting to violence and frequently to the ghettoization of the infected.

The player takes on the role of the "Doctor," who suffers from amnesia after waking up from a sleep that caused memory loss. The Doctor commands a team of "operators" belonging to Rhodes Island Pharmaceuticals, a pharmaceutical organization equipped with military support squads. The team’s mission is to find a cure for the disease while simultaneously suppressing attacks from the anarchist military movement, Reunion Movement, which seeks to avenge the victims of the disease who have died or been threatened by the government, as well as to claim the rights of the infected.
